I've got the same faults on mine, it seems that the heater element fails eventually. There are two fuses, one for each side. The passenger side heater is in the front passenger side fusebox, and the drivers side is on the drivers side fusebox. Both fuseboxes are on the end of the dashboard.

You have to replace the whole assembly, its not cheap and the front seats and carpet need to come out to even get to them.
