I found two M8 threaded clevises that could work, instead of getting a £10+ brake booster clevis and having to cut it down and tap it to M8.

One may be too long:

Came accross a potential major problem, with my specific clutch pedal mod at least, the boot of the brake master cylinder piston is quite wide and has to move through the area where I was going to weld on the clutch master cylinder and over centre spring mounting plate. Oops!

Back to the drawing board.

I've seen that snag, I'm leaning towards having a pedal cluster made from scratch.

Perhaps you are right, a complete custom job may be in order.
One more potential frankenstein solution, a BMW E46 clutch pedal has a nice thin looking design for the master cylinder and over centre spring mounting, maybe it could work. Shame the master cylinder is plastic, thats BMWs for you

Just a quick heads up. Another issue I am concerned about is the safety and comfort of having 3 pedals in the very narrow D2 footwell. There is enough room for the clutch pedal without the foot rest, but this may make for uncomfortable driving having to always rest the left leg on the floor in front of the clutch pedal.
There may be some space to be found on the right, by reducing the pointless size of the fuse box cover. I drove around yesterday without this cover and it felt a lot more roomy down there. A narrower cover may be in order, and that may allow the accelerator pedal to move 1-2cm to the right, and the brake pedal as well. Maybe it is just possible to retain a narrow footrest?

The foot rest is an integral part of the shell, and can't be removed, or bashed flat with a mallet. At least I'm not going that route. Still looking at a two-pedal option.

If you flatten the foot rest do you not run the risk of fouling the starter motor?

S'ok, it'll be a manual, so just remember to park on a hill and you can bump start it....

Possibly, not dead sure though. The carpet would be pretty baggy there though, and for some reason, I'm not really keen on compromising the aluminium skin, only to find out the starter motor's closer to the trans tunnel than you think.

the foot rest I'm talking about is a piece of plastic that takes about 5 seconds to remove, there is nothing behind it. If you add a third pedal without modifying the brake or gas pedal, the plastic foot rest will have to go. Then there will be just enough room for a clutch pedal. Its the big plastic thing to the left of the picture.

My issue is, if you have to keep your foot on the floor in front of the clutch pedal, it may get uncomfortable as you cant stretch out your leg, and also reduce your reaction time as you have to lift your foot all the way up onto the clutch pedal. If you had a foot rest like in a LHD car you could just move the foot to the side and you would be right on the clutch pedal.
