Could the air pipe be blocked or trapped somewhere, or the bellows unit be jammed?

You have vacuum actuator that moves the rod. For it to work you need vacuum going to the actuator and electricity that operates the valves at the actuator and change direction of movement. You need to check weather there is vacuum and electrical signal the the actuator when you lock/unlock to see if it is vacuum, electrical problem or faulty actuator.

How do I get the actuator out? I can't see an obvious attachment

2 screws hold it in place but they are invariably covered in that awful hard black plasticky stuff that you will see - almost like tar.
