I spent the weekend stripping out and rebuilding the inlet manifold. Put it back and I'm still getting the same problems. This is disheartening to say the least. I'm not sure which direction to go now. I'll do another scan and see what we get. I think I may do a smoke test to find out for sure there are no leaks in the inlet system. After that I'm not sure what else to do except seek more advice.
