Please see the attached pdf's. Had to split in 2 as in one file it is 196.3k and max allowed here is 195k for pdf's. If it does not work drop me PM with Your e-mail and I will send it to You.
Mike You can get anything that's on the screen by pressing "print screen" button, then paste into paint and save picture. Or get CutePDF program(free) like me. That's a virtual printer which saves anything You can print to pdf files. Together with iCopy(also free) You can also scan straight to pdf.
