There are two separate pumps, so a failed pump is an option. Another option could be completely blocked nozzles, due to a build up of grime. If it's leaking pipework, chances are you'd see a puddle of water under the front bumper when you try washing the headlights.

Would a scan show anything?

Highly unlikely, and in terms of physical access to the pumps to test them, that means removing the rear part of the driver side wheel arch, and you could try putting 12v across them.

Thanks for the help. I'm thinking the problem might be cracked pipework as there was a bit of a puddle under the nearside bumper I've since discovered. I'm struggling to find a diagram of the pipework routing so looks like I'll just have to get in there and have a proper look.

Here's what the pipework looks like. Chances are that if the pipework is still intact, then the leak could be occurring at #12, the o-ring (N90303801). The only other spot might be at the connection from pipe #1 to pipe #5, but I don't know if the diagram is accurate, with the joint being in the centre. Since it's tucked up inside the bumper cover, water could be running to the passenger side. Time to fill the tank, use the headlight washers, and get under the front of the car.
